The licensing inspector conducted an unannounced monitoring inspection on Monday, June 13, 2022 from 11:45am to approximately 2:15pm. There were a total of 49 children in care in the direct care of nine staff members. During the inspection, the children and staff were observed participating in a variety of activities. Staff were observed having positive interactions with the children. All areas of the facility used by the children were inspected. The center is equipped with toys and supplies and items were available to the children. The required postings were reviewed and found to be in compliance. Transportation is provided and two vehicles were inspected. Medication is administered when required and medications were reviewed. During the inspection, five children?s records and six staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-780-130-E Description: Based on a review of five children?s records and interview, the center did not obtain documentation of additional immunizations once every six months for one child under the age of two years.
Evidence: 1) Child #4 was under 2-years-old at the time of enrollment on 03/08/21. The record did not contain documentation of additional immunizations once every six months. The most recent immunizations in the record were dated 02/15/21. During interview, a member of management confirmed the records did not contain additional immunizations.Plan of Correction: The center will obtain required immunization updates moving forward.
Standard #: 8VAC20-780-160-A Description: Based on a review of six staff records and interviews, the center did not ensure two staff members had documentation of a negative tuberculosis (TB) screening at the time of employment and prior to coming into contact with children.
Evidence: 1) The TB screening in the record for Staff #2, hired on 06/02/22, was dated 06/05/22. 2) The TB screening in the record for Staff #3, hired on 05/24/22, was dated 06/03/22. 3) During interviews, a member of management reported Staff #2 and Staff #3 did not have documentation of a negative TB screening at the time of employment and prior into coming into contact with children.Plan of Correction: Moving forward, staff will submit TB screenings prior to hire.
Standard #: 8VAC20-780-280-H Description: Based on observations at the center on 06/13/22, the center did not ensure purses were not accessible to children.
Evidence: 1) The licensing inspector observed a staff members? purse on a child?s table in the Fours classroom. The children were observed playing near the table and the purse was accessible to the children.Plan of Correction: Corrected on site. Staff reminded to lock purses and not accessible to children.
Standard #: 8VAC20-780-520-C Description: Based on a review of documentation, observations, and interviews, the center did not ensure that when a diaper cream was used, it was used with written parent authorization.
Evidence: 1) A diaper cream belonging to Child #4 was observed at the center on 06/13/22. 2) During interview, a member of management reported the center did not have written parent authorization for the diaper cream. 3) Staff interviewed reported the diaper cream was applied to Child #4.Plan of Correction: The form will be requested today. In the future, parents and staff will be reminded that the required forms must be on file before diaper cream/ointment is applied.
