Comments:
The licensing inspector conducted an unannounced Subsidy Health and Safety inspection on October 26, 2023 from approximately 11:30 AM to 1:30 PM. There were 26 children in attendance and a total of six staff supervising. A total of five children and six staff records were reviewed. A code compliance inspection was conducted on the same date as the SHSI supplemental inspection.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice, sign and returned it to me within five business days from today. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Description: Based on review of five children's records, the vendor did not ensure the records of three children?s records contained the required information.
Evidence:
1. The records for Child #1, Child #2, and Child #3 did not contain the address for the listed emergency contact.

Description: Based on review of six staff records and interview, the vendor did not ensure four staff records contained the required information.
Evidence:

Description: Based on review of six staff records, observation and interview, the vendor failed to obtain satisfactory results of the Virginia Child Protective Services Central Registry check for one staff member.
Evidence:

2. The staff were observed working with children in the center during the inspection.

Description: Based on review of six staff records, the vendor did not ensure three staff were evaluated by a health professional and be issued a statement that the individual is determined to be free of communicable tuberculosis (TB) within the required timeframe.
Evidence:
1. The record for Staff #4 (Date of Employment 04/28/2023) contained a TB screening dated 01/18/2023.
2.. The records of Staff #5 (Date of Employment 06/12/2023), and Staff #6 (Date of Employment 09/25/2023) did not contain documentation of a TB screening.
3. Staff #4, Staff #5, and Staff #6 were observed working in the center.
4. The documentation of the TB screening is required to be obtained within the last 30 calendar days of the date of employment and be signed by a physician, physician?s designee, or an official of the local health department.

Description: Based on the review of five children's records, the vendor did not obtain documentation that each child had received the immunizations required by the State Board of Health before the child attended the center.

Description: Based on review of six staff records, interview and observation, the vendor did not ensure all staff who work directly with children obtained current certification in first aid appropriate to the ages of children in care within 90 days of the date of employment.

Infant #1 (six months of age) was observed sleeping in the crib. A boppy pillow was placed in the crib, underneath the head of Infant #1.
