Comments:
A renewal inspection was conducted on May 6, 2024 from approximately 9:15 AM to 1:40 PM. There were 25 children present, with nine staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies and nutrition. A total of seven child records and nine staff records were reviewed. The center did not have any medications to administer or review. The provider stated that transportation is provided, and the vehicles and transportation documents were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the plan of correction and the date to be corrected for each violation listed on the violation notice and return it to the department within 5 business days from receipt. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s); 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Standard #: 8VAC20-770-40-D-6
Description: Based on a review of staff records, the center failed to ensure that a criminal history background check and a central registry check was not more than 90 days prior to employment.

Evidence:
The record for staff #7 (employed: 4/29/24) had documentation of the criminal history background check dated 12/15/23. Staff #7 had documentation of central registry findings dated 01/11/24. A member of management verified staff #7?s first day of employment.

Description: Based on record review and interview, the center did not ensure all staff records had documentation of a completed sworn statement or affirmation prior to the first day of employment.

Evidence:
1. The record of staff #9 (employed: 02/02/24) did not have documentation of a complete sworn statement prior to hire.
2. A member of management stated that staff #4 and staff #5 did not complete a sworn statement prior to hire.

The record for staff #6 (employed: 8/21/23) did not have documentation of central registry findings. On 12/21/23, the licensing inspector observed documentation that staff #6 had submitted the required central registry to the department of social services.

Description: Based on record review, the center did not ensure each record contained additional immunizations once every six months for children under the age of two years.

Evidence:
The record for child #2 (birth date: 5/13/23) had documentation of immunization at time of enrollment on 06/22/23. No other immunization records were in the record for child #2.

Description: Based on record review, the center did not ensure that all staff complete the Virginia Department of Education-sponsored orientation course within 90 calendar days of employment.
Evidence:
The records for staff #3 (employed: 1/02/23) and staff #5 (employed: 08/22/22) did not contain documentation of the completed Virginia Department of Education sponsored 10-hour orientation course which must be completed within 90 calendar days of employment.
2. A member of management confirmed that the two staff members had not completed the course and/or the documentation could not be located.

Description: Based on observation, the center failed to ensure that equipment of the center shall be maintained in a safe condition.

Evidence:
? In room #5, an infant was observed sitting in a bouncer seat which had been placed in a crib. The U.S. Consumer Product Safety Commission recommends bouncer seats to always be used on the floor, never on a countertop, table, or other elevated surface. Warning labels indicate use bouncer only on the floor.

Description: Based on observation, the center did not ensure all hazardous substances such as cleaning materials insecticides and pesticides, were kept in a locked place using a safe locking method that prevents access by children.

Evidence:
In room #1 and room #2 a container labeled bleach and water was sitting on top of the children?s cubbies.
In room #4, Glass Plus was sitting on the countertop.

Description: Based on observation, staff interview, and staff files, the facility failed to have at least one staff member who meets the qualifications of a program leader regularly present.
Evidence:
In room #1, staff #5 stated she was the lead teacher, staff #5 does not have program leader qualifications documented in the record.
In room #2, staff #9 stated she was the lead teacher, staff #9 does not have program leader qualifications documented in the record.
In room #3, staff #2 stated she was the lead teacher, staff #2 does not have program leader qualifications documented in the record.
In room #4, staff #3 stated she was the lead teacher, staff #3 does not have program leader qualifications documented in the record.

Description: Based on observation, the center did not ensure that cots used by children during the designated rest period shall have linens consisting of a top cover and a bottom cover or a one-piece covering which is open on three edges.

Evidence:
In classroom #2, six children were on cots that did not have a bottom cover.
In room #4, four children were on cots that did not have a bottom cover.

Description: Based on record review, the licensee did not ensure there were a minimum of two shelter-in-place practice drills, yearly.
