Thank you for your time and assistance with the unannounced monitoring inspection conducted today, November 26. 2019. The on-site visit was from 10:30 a.m. to 1:30 p.m. On this date, there were 29 children present in two Montessori classrooms and in the care of six staff members. The total enrollment for the school was 30 children. There were five staff members' records reviewed and five children's records reviewed. Five staff members and four children were interviewed. Observations were made in both classrooms. The medications and medication paperwork were reviewed for three children. All food was provided by the parents. All interactions between staff and children were age-appropriate and positive. Some of the items reviewed and observed included: updated emergency preparedness plan, annual fire and health inspections, injury reports, documentation of evacuation and shelter-in-place drills, first aid kit, emergency supplies, warm water temperature, outdoor play, Montessori work activities, musical activities, transitions, hand washing and bathroom procedures, lunch and nap time.

Standard #: 22VAC40-191-60-C-2 Description: Based on review of five staff records on 11/26/2019 and confirmation from staff, there was no completed Search of the Central Registry for one staff.
Evidence:
1. The record for Staff Member #2 did not contain results of a Search of the Central Registry. Date of hire was 09/27/2019.
2. A staff member called the Office of Background Investigations and was told the request for a search had not been received.Plan of Correction: Within 10 days, the director will overnight a new request for a Central Registry Search. The director will send proof that the request has been made to the licensing inspector. If results are not received within 30 days, the center will follow-up with the Office of Background Investigations.
