Comments:
An unannounced monitoring inspection was conducted on 5/13/24 from 9:34 a.m. to 12:06 p.m. with the Director. There were 90 children in care, ranging in age from three months to five-years-old, supervised by 15 staff. The children were observed in structured activities. Five child records and six staff records were reviewed. The Center has 28 staff with current certification in CPR and First Aid, and 30 staff trained in Daily Health Observation Training. Five medications and authorization forms were reviewed, and the center has five staff current in Medication Administration Training (MAT). Required postings were observed. The attendance, menu, and emergency drill log were reviewed. The first aid kit, flashlight, and battery-operated radio were observed. Areas of non-compliance are identified in the Violation Notice. Standard #: 8VAC20-780-80-A
Description: Repeat Violation

Based on review of the written attendance record, the center did not ensure they maintained a written record of daily attendance for each group of children documenting arrival and departure of each child in care as it occurs.

Evidence: In the Preschool class there were ten children present in the classroom, the written record reflected there were nine.

Plan of Correction: The staff changed the written attendance the day of inspection.

Standard #: 8VAC20-780-330-B
Description: Repeat Violation

Based on observation and measurement, the facility did not ensure that where playground equipment is provided, resilient surfacing shall comply with minimum safety standards.

Evidence: on the ?big? playground there was approximately two inches of mulch observed at the base of the composite play structures slide exits, fall zones and six swings.

Plan of Correction: Put a work order to put the new mulch on the playground.

Standard #: 8VAC20-780-560-G
Description: Repeat Violation

Based on observation, the center did not ensure that when food is brought from home that it is labeled in a way that identifies the owner.

Evidence: There was a lunch box in the Toddler and Montessori A class with no name.

Plan of Correction: Names were placed on both lunch boxes on the day of the inspection.
