Comments:
An unannounced monitoring inspection was conducted 0n 5/30/2024 from 11:15am-1:30pm. The physical plant, 4 staff records, 5 children?s records, evacuation drills, injury reports, and policies were inspected. Children were observed engaging in imaginative play outside where they pretended to make a campfire and search for supplies. Areas of non-compliance are identified in the Violation Notice.

Standard #: 22.1-289.011-F
Description: Based on record review, required documents were not posted in a conspicuous place on the licensed premises.

Evidence:
1. The findings from the most recent licensing inspection were not posted on the day of the inspection.

Plan of Correction: The most recent inspection results will be posted.

Standard #: 8VAC20-780-160-C
Description: Based on record review, 1 of 3 staff did not obtain a tuberculosis screening every two years.

Evidence:
The most recent tuberculosis screening documented in the record for Staff #2 was dated 1/17/2022

Plan of Correction: An updated tuberculosis screening will be obtained.

Standard #: 8VAC20-780-60-A
Description: Based on record review, 2 of 5 children?s records did not contain all required information.

Evidence:
1. The record for Child #2 did not contain documentation of the name address and telephone number of 1 of 2 individuals to contact in an emergency if the parents cannot be reached.
2. The information for the child?s mother was listed on the documentation requesting emergency contacts.

Plan of Correction: The information will be requested from the parents.
