Comments:

An unannounced, on-site monitoring inspection was initiated on 7/17/2024 and completed on 7/17/2024, as a part of the licensure period. The on-site inspection began at 10:13AM and ended at 12:16PM. The inspector reviewed compliance in the areas listed above. There were 85 children present and 17 staff. The inspector reviewed 3 children?s records and 2 staff records on-site. This inspection included ? ?document review (i.e. Injury logs, policies and procedures, emergency drill logs, medication administration logs, authorization forms), ?tour of the facility, ?interviews, ?observations (this would include observations of photos, recordings, etc.) Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on (date). A POC submitted after this date will not appear on the public website.

Standard #:	8VAC20-770-60-C-2
Description:	Employees must have a Central Registry check by end of the 30th day of employment. Staff #2 was hired on 6/1/2024 and did not have Central Registry check results during the inspection on 7/17/2024.
Plan of Correction:	Provider has submitted for central registry results.
Standard #:	8VAC20-780-330-B
Description:	Where playground equipment is provided, fall zones shall not include barriers for resilient surfacing. There was a small wooden table located approximately two feet from the triangle climbing structure and was within the fall zone during the inspection on 7/17/24.
Plan of Correction:	Provider has moved wooden table from within fall zone of climbing structure.
Standard #:	8VAC20-780-440-B
Description:	Cribs, cots, rest mats and beds shall be identified for use by a specific child. Licensing inspector observed 11 cots not labeled in the 2-3 year old classroom during the inspection on 7/17/24.
Plan of Correction:	Provider has labeled all cots.
Standard #:	8VAC20-780-500-B
Description:	Disposable diapers shall be disposed of in a leakproof or plastic lined storage system that is either foot-operated or used in such a way that neither the staff member?s hand nor the soiled diaper touches an exterior surface of the storage system during disposal. Licensing inspector observed a foot operated trashcan in the 16-24 month room that would not open by pressing the foot pedal. Staff confirmed diapers were disposed of in a trash can under the sink where the cabinet door had to be opened by hand.
Plan of Correction:	Provider has replaced foot operated trashcan with a new one that opens by pressing the foot pedal.
