During inspection the following required information for the assistant present and caring for children was not available: Date of Employment or volunteering, Emergency contact, documentation of 1 of 2 required references, Background checks , Current TB screening and orientation to positionPlan of Correction: This was an emergency because my usual assistant traveled out of town. I will work to get the information.
