An unannounced complaint investigation was initiated on September 23, 2019 in response to allegations received by the Division on September 9, 2019 regarding physical plant issues. Additional documentation was received on September 25, 2019. Based on observation, staff interviews, and documentation reviewed, the preponderance of evidence gathered during the investigation does not support the allegations.
