Comments:
An unannounced renewal inspection was conducted on Tuesday, August 20, 2019 to review to review the center's compliance with licensing standards. The inspector was on site from approximately 10:15am to approximately 1:30pm. The center's director assisted the inspector throughout the inspection. The census for today's inspection consisted of 15 children in the direct care of 3 staff. The children were engaged in a variety of activities during the inspection. The school age children were observed playing with peers and interacting with staff. They were observed during a teacher-led activity where they sang songs and played games as a group. The preschool children were observed during free-play. The children were later observed taking bathroom breaks, washing hands, and as they prepared for their designated rest/quiet time. The menu was posted and it consisted of turkey and cheese on sub roll, fresh nectarine, orange juice, and milk. All areas of the center including classrooms, hallways, kitchen, bathrooms, and one vehicle were inspected. The center was equipped with toys and supplies and items were available to the children.

The following information was reviewed: injury reports, daily sheets, daily attendance, practiced emergency drill logs, parent agreements, and staff training information. The center's first aid kit and emergency supplies were inspected. The center administers emergency medications. Per the director, there are no medications on site at this time. The required information was posted.

During the inspection, six children's and three staff records were reviewed.

Violations:
Standard #: 22VAC40-185-130-A
Description: Based on a review of six children's records and interview on 08/20/2019, the center did not obtain documentation that one child had received the immunizations required by the State Board of Health within the required time frame. Evidence: 1) Child #6, who was enrolled on 03/11/2019, did not have documentation of immunizations prior to attendance. The immunization record was dated as signed by the physician on 03/26/2019. 2) During interview, the director confirmed there was no other immunization documentation for Child #6 prior to 03/26/2019. The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.

Plan of Correction: Children will not be starting unless the entire file is complete. The director will audit each file to make sure they are complete.

Standard #: 22VAC40-185-40-H
Description: Based on a review of records and interview on 08/20/2019, the center was not able to demonstrate it was covered by public liability insurance for bodily injury for each center site with a minimum limit of at least $500,000 each occurrence and with a minimum limit of $500,000 aggregate. Evidence: 1) During the inspection, the liability insurance declaration documentation presented by the director had the policy as expired on 05/23/2019. 2) During interview, the director stated the policy was renewed, but it was not available for review while the inspector was on site.

Plan of Correction: The owner requested a copy of the current liability insurance to be sent to the center and it will be forwarded to the inspector.
