Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on 12/3/20 and concluded on 12/4/20. The director was contacted by telephone to initiate the inspection. There were 4 children present with 2 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 2 staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record review and interview, the center failed to obtain all the required information for one out of two children's records reviewed.

Evidence:

1. The record for child 1 had N/A documented for the father's information.
2. The director stated the father's name is listed on the birth certificate and the center does not have custody papers. The director contacted the mother of child 1 who stated she does not have custody papers.
3. The director stated the father's name is listed as a person not authorized to pick up child 1.

Plan of Correction: The director will have the mother of child 1 add the father's information to child 1's record. The father's name will be removed as an individual that is not authorized to pick up child 1 unless custody papers are provided. In the future the director will review children's records upon receipt to ensure all information is completed. If one of the parent's information is not documented and the name is on the birth certificate the parent will be asked to provide custody papers or complete the paperwork.
