Comments:
A subsidy health and safety (SHSI) inspection was conducted June 6, 2024. There were 14 children present with four staff supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations.
Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-600-F
Description: Based on review of four staff records, the center did not ensure one staff completed, in
addition to preservice, and orientation training, annually attend at least 16 hours of training to include the department?s health and safety update course.
Evidence:
1. The record of staff #3, employed 11/21/2022, did not contain documentation of the health and safety update training exceeding the annual requirement.
2. The director stated staff #3 had not completed the training.

Plan of Correction: Per the director: I will have the staff complete the training.
