Comments:
A renewal inspection was conducted on May 15, 2024 between 9:55am and 11:35am. There were 3 children present, with 2 caregivers supervising. A total of 5 child records, 2 caregiver and household member records were reviewed. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies and nutrition. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations:
Standard #: 8VAC20-800-760-A-1
Description: Based on observation, and interview the first aid kit did not contain the minimum requirements.

Evidence:
1. There were no tweezers available at the time of inspection.
2. There were no triangular bandages available at the time of inspection.
3. Caregiver #1 acknowledged that she could not locate the minimum required items.

Plan of Correction: I have them but I will find them today and place them back in my first aid supplies.

Standard #: 8VAC20-800-780-B
Description: Based on observation, the provider failed to review annually with the parent the emergency contact information required to ensure the information is correct. Repeat violation.

Evidence:
Child # 3 (DOE 10/24/22) has not had an annual review since the date of enrollment.

Plan of Correction: I will have the parents review the information when they return on Monday.

Standard #: 8VAC20-800-830-B
Description: Based on interview and observation, shelter-in-place procedures were not practiced twice a year.

Evidence:
There was no documentation of the shelter-in place procedure being done in the past year.

Plan of Correction: I will conduct a shelter-in-place tomorrow and be sure to do them twice a year.

Standard #: 8VAC20-800-100-A
Description: Based on a review of child records, the provider did not obtain documentation of a physical examination within 30 days after the first day of attendance. Repeat violation.

Evidence:
Child #4 (DOE 2/7/23) record did not have documentation of a physical examination at the time of the inspection.

Plan of Correction: I will call the parents today and tell them I need the physical within 2 weeks.

Standard #: 8VAC20-800-240-A
Description: Based on observation, areas and furnishings of the family day home were not maintained in a clean, safe, and operable condition.

Evidence:
The changing pad was visibly cracked, and could snag children's skin.

Plan of Correction: I've ordered a new pad, and it is schedule to be delivered within 2 weeks. I will cover the pad until I get the new one.

Standard #: 8VAC20-800-680-B
Description: Based on observation, caregivers did not ensure that children's hands were washed with liquid soap and warm running water after diapering.

Evidence:
Caregiver #1 did not wash the hands of child #1 after changing their diaper.

Plan of Correction: I will ensure children's hands are washed after toileting. Violation was corrected during inspection.

Standard #: 8VAC20-800-690-D
Description: Based on observation, the diapering surface was not cleaned and sanitized after each use.

Evidence:
Caregiver #1 used a baby wipe to clean the diapering surface after changing child #1.

Plan of Correction: I will clean the changing diaper surface with soap & water, and use a bleach solution to disinfect. Violation corrected during the inspection.
