Comments:
An unannounced renewal inspection was conducted on 5/28/24 from 11:12am-12:05pm. There were 6 children (16 points) directly supervised by 2 caregivers. The home, caregiver records, children?s records, household member?s records, evacuation drills, emergency supplies, and policies were reviewed. There were no medications on site today. Children were observed playing freely and eating lunch. There was an assortment of age appropriate materials for children. Areas of non-compliance were found and identified in the Violation Notice.

Violations:
Standard #: 8VAC20-800-800-B
Description: Based on caregiver interview and record review, the provider did not document or review the emergency plan at least annually and did not update the plan as needed.

Evidence:
The provider last reviewed and documented the review on 2/20/2019. The provider confirmed that the plan was not updated annually.

Plan of Correction: The provider reviewed and updated the emergency plan during the inspection.

Standard #: 8VAC20-800-60-B
Description: Based on record review, the provider did not maintain all the required information for 2 out of 5 enrolled children.

Evidence:
1. Child #1 was missing the address for one of the emergency persons to contact in case a parent cannot be reached.
2. Child #3 was missing documentation of the annual review of the child's emergency contact information.

Plan of Correction: The provider will ask parents for the missing information and update the files today.

Standard #: 8VAC20-800-240-A
Description: Based on observations, areas outside of the family day home was not maintained in a safe and operable condition.

Evidence:
1) Multiple tripping hazards were observed on the brick patio area located in the outdoor play area. Several bricks were protruding, creating potential trip hazards.
2) Long broken wires with sharp edges were observed next to the door that provides access to the playground. The wires were accessible to the children.

Plan of Correction: The provider moved a trash can in front of the hazardous wires so the children do not have access to it. The provider will contact a contractor to remove the multiple trip hazards on the brick patio and will use the neighborhood playground until it is fixed.
