Comments:
An unannounced monitoring visit and facility tour was conducted on 6/6/24. At the time of inspectors' arrival there were 60 infant to preschool aged children in care with 10 staff members and 2 administrators. Children were observed interacting with staff, engaging in educational activities, nap, lunch and hand washing. A sample of 5 children's records and 5 staff records were reviewed. Injury reports were also reviewed. Director reported that medications are not being administered at this time. Areas of noncompliance are identified on the violation notice and were discussed with the director in an exit meeting at the conclusion of this inspection.

Standard #: 8VAC20-780-130-A
Description: Based on record review and interview it was determined the center did not obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.
Evidence:
1. The record for child #5, present during this inspection, did not contain documentation of immunizations.
2. Staff confirmed this documentation was not available during this inspection.

Plan of Correction: Director requested information from parent.

Standard #: 8VAC20-780-140-A
Description: Based on record review and interview it was determined the center did not ensure each child had a physical examination by or under the direction of a physician.
Evidence:
1. The record for child #5, present during this inspection, did not contain documentation of a physical exam.
2. Staff confirmed this documentation was not available during this inspection.

Plan of Correction: Director requested information from parent.

Standard #: 8VAC20-780-60-A
Description: Based on record review and interview it was determined the center did not ensure the record for each child enrolled contained all the required information.
Evidence:
1. The record for child #3 did not contain one of the two required emergency contacts.
2. The record for child #5 did not contain all the required information for the emergency contacts.
3. Staff confirmed this information was not available during this inspection.

Description: Based on record review and interview it was determined the center did not ensure all staff completed the Virginia Department of Education-sponsored orientation course within 90 calendar days of employment.
Evidence:
1. There was no documentation of completion of the Virginia Department of Education-sponsored orientation course for staff #1(hired 5/8/23.
2. Staff confirmed this documentation was not available during this inspection

Plan of Correction: Director asked staff to complete if needed and provider documentation of completion.

Standard #: 8VAC20-780-270-A
Description: Based on observation and interview it was determined the center did not ensure that all areas and equipment were maintained in a clean, safe and operable condition.
Evidence:
1. The soft foam activity mat in the infant room was torn exposing the soft foam core material.
2. Staff confirmed this mat was torn.

Plan of Correction: The mat was removed during the inspection.
