The inspector conducted an unannounced mandated inspection from 10:10 a.m. until 12:20 p.m. There were two children in care with one staff present at the beginning of the inspection; the director arrived approximately 20 mins. later. The center had lost water for the day and had contacted parents to pick up their children. Five children's records were reviewed and two staff records were reviewed. Children were observed playing and watching a video and eating lunch. Mo medications were being administered. Please contact your inspector at 276-698-9981 with any questions.

Standard #: 22VAC40-185-70-A Description: Based on record reviews, the center failed to complete the required training for each staff person.
Evidence:
1. The inspector reviewed two staff records. One of two records (record #2) did not have orientation documentation.Plan of Correction: Director found the orientation and placed it in the file.
Standard #: 22VAC40-185-240-D-5 Description: The center failed to have a staff person with daily health observation training at the center at all times.
Evidence:
1. The inspector arrived at the center, and the staff person (#2) did not have daily health observation training.Plan of Correction: The director and staff member will take the on-line daily health training.
Standard #: 22VAC40-185-270-A Description: The center failed to maintain the areas of the playground in a safe condition.
Evidence:
1. The inspector observed two plastic toys on the playground that were crumbling.Plan of Correction: Director will remove the equipment.
Standard #: 22VAC40-185-340-C Description: Based on observation, the center did not have on site two staff members at all times.
Evidence:
1. The inspector arrived at the center at 10:10 a.m. There was one staff person at the center. The director was called and arrived approximately 20 minutes later.Plan of Correction: The director will make sure there are two staff people at the center at all times.
Standard #: 22VAC40-185-530-A Description: Based on record review and interview, there was not a trained staff person in first aid/cpr on site.
Evidence:
1. The inspector asked to see first aid/cpr training documentation and it had expired 01/11/2019.Plan of Correction: The director and staff will get the training later on this month
