A renewal inspection was initiated on 7/25/2022 and concluded on 7/25/2022. There were 14 children present with three staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and three staff records were reviewed. The inspection started at 9:50 am am and concluded at 12:05 pm.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-780-140-A Description: Based on review of records the center failed to have a physical examination before child's attendance or within 30 days after the first day of attendance.
Evidence:
Child #2 (start date missing) and 5 (start date 1/30/2020) do not have a physical on file.Plan of Correction: The director will have the parents bring in the physicals for these children.
Standard #: 8VAC20-780-60-A Description: Based on record reviews the center failed to ensure that all required information was contained in the children's records.
Evidence:
1. Child records #2 & 3 did not have addresses of the emergency contact listed.
2. Child records #2 & 3 did not have the first date of attendance listed.
3. Child record #1 listed a custodial parent as not allowed to pick up the child, however, there was no legal paperwork in the file.Plan of Correction: The director will have the parents update the children's files.
Standard #: 8VAC20-780-260-A Description: Based on review of requirements subsequent to initial licensure the center failed to have an annual fire inspection.
Evidence:
The last fire inspection report is dated 5/18/2021.Plan of Correction: The director will contact the fire marshal and request another inspection
