Comments:
A monitoring inspection was initiated on 2/1/2024 and concluded on 2/1/2024. There were ten children present with three staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and two staff records were reviewed. The inspection started at 1:00 pm and concluded at 2:45 pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the ?plan of correction? and date to be corrected? for each violation cited on the violation notice and return it to me within five (5) business days from today (2/2/24). You will need to specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring gain; and 3) person or person(s) responsible for implementation.

Description: Based on observation the center failed to ensure that the license and any other documents required by the Superintendent shall be posted in a conspicuous place on the licensed premises.
The most current violation notice (11/1/23) was not posted at the center.

Standard #: 8VAC20-780-280-B
Description: Based on observation the center failed to ensure that hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children.
Evidence:
Located in the preschool/school age area is a storage closet. The door to this closet was unlocked. Inside the closet on the floor and on the shelves was various cleaning products such as, The Works, OdoBan, Foaming Bubbles, Glass cleaner and Pine sol. Children have access to this door.

Standard #: 8VAC20-780-280-D
Description: Based on observation the center failed to ensure that cleaning and sanitizing materials shall not be located above food, food equipment, utensils or single-service articles and shall be stored in areas physically separate from food.
Located on the table in the kitchen was two boxes with pizza inside, cupcakes and various other food items. On the end of the table beside the pizza boxes was a bottle of disinfecting spray and a container of disinfecting wipes.

Standard #: 8VAC20-780-500-B
Description: Based on observation the center failed to ensure that a nonabsorbent surface for diapering or changing shall be used. Disposable diapers shall be disposed in a leakproof or plastic lines storage system that is either foot-operated or used in such a way that neither the staff member's hand not the soiled diaper touches an exterior surface of the storage system during disposal.
Evidence:
1. The diaper pad on the changing table had several tears.
2. Located in the diapering room is a diaper Genie that is not foot operated. The lid has to be touched my the hand in order to open the lid.

Standard #: 8VAC20-780-570-B
Description: Based on observation the center failed to ensure that bottle fed infants who cannot hold their own bottles shall be held when fed. Bottles shall not be propped or used while the child is in his designated sleeping location.
Evidence:
During naptime two infants (11 and 13 months old) were drinking their bottles in the cribs.
