A complaint inspection was initiated on 4/11/2024 and concluded on 5/28/2024. There were three children present with two staff supervising. The inspector reviewed compliance in the areas of staffing and supervision, special care. A total of two staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 22.1-289.035-B-4 Complaint related: No Description: Based on observation the center failed to ensure that an out of state Sex Offender Registry completed for each state an individual has resided in the past 5 years prior to the first day of employment/service. Criminal History name check is required prior to he first day of employment or service.

Standard #: 8VAC20-780-160-A Complaint related: No Description: Based on record review the center failed to ensure that each staff member and individual from an independent contractor shall submit documentation of a negative tuberculosis screening at the time of employment and prior to coming into contact with children.
