Comments:
An unannounced monitoring inspection was completed on 11/21/2019 from 10:30 A.M.-2:15 P.M. At the time of the inspection 95 children were in care with seventeen staff and one administrator present. Nine children records, nine staff records, three medications and required paperwork, over-the-counter skin products and required documentation, four injury reports, first aid kits, emergency supplies, health inspection, fire inspection, emergency plan, and posted required information were reviewed. Children were observed during infant feeding, diapering, transition to outdoor play time, arrival back from a field trip, behavioral guidance, story time, handwashing, lunchtime, and naptime. The exceptions to compliance are noted on the violation notice.

Violations:
Standard #: 22VAC40-185-510-E
Description: Based on observation and interview, the center failed to ensure that medication was labeled with the child's name, the dosage amount, and the time or times to be given.

Evidence:
1. In Classroom #3 there was a bottle of Motrin and a bottle of Children's Tylenol that was not labeled with the child's name, dosage amount, and the times to be given.
2. Staff verified that the medications were not labeled with the child's name, dosage amount, or times to be given.

Plan of Correction: Staff immediately labeled the medication. Administration will review with all staff the requirements regarding medications.

Standard #: 22VAC40-185-520-A
Description: Based on a review of over-the-counter skin products, the center failed to ensure that over-the-counter skins products shall not be kept beyond the expiration date of the product.

Evidence:
1. In Classroom #4 there was a tube of Boudereaux Butt Paste that had expired in 09/2018.
2. In the restroom outside of Classroom #5 there was a tube of desitin diaper cream that had expired in 01/2019.
3. Staff verified that the diaper ointments had expired.

Plan of Correction: Staff will give the diaperer ointments back to the parents at pick-up time. Administration will review medication of over-the-counter skin products with staff.
