2. Child #3 does not have the name, address and telephone number of each custodial parent's place of employment.Plan of Correction: The Provider will ask the parents for this.
Standard #: 22VAC40-111-90-C Description: Based on file review, 2 out of 5 children's records reviewed does not have additional immunizations every six months up to the age of 2 years.
Evidence
1. Child #3 is 21 months old. The most recent immunization record was updated when the child was 14 months old.
2. Child #4 is 30 months old. The most recent immunization record was updated when the child was 16 months old.Plan of Correction: The Provider will ask the parents for the most recent shot record.
Standard #: 22VAC40-111-280 Description: Based on observations, the Provider failed to keep all cleaning agents and disinfectants stored away from areas accessible to the children in care.
