An unannounced, on-site renewal inspection was initiated on 7/3/2024 and the inspection concluded on 7/3/2024. The on-site inspection began at 11:00AM and ended at 12:30PM. The inspector reviewed compliance in the areas listed above. There were 4 children, 1 caregiver and 2 volunteers present. The inspector reviewed 4 children?s records, 2 caregiver records and 1 household member record on-site on 7/3/2024. This inspection included document review, tour of the facility and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 7/9/2024. A POC submitted after this date will not appear on the public website.

Standard #: 22.1-289.036-A Description: Household members 18 years of age and older must have repeat background checks conducted every 5 years. Household member 1 did not have 5 year repeat check results during the inspection on 7/3/2024. Plan of Correction: Provider will submit for 5 year repeat checks
Standard #: 8VAC20-800-100-A Description: The provider shall obtain documentation of a physical examination prior to a child?s attendance or within 30 days after the first day of attendance. There was no physical exam record available for Child 2 and Child 3 who were provided care in July 2024. Child 2 and Child 3 have been enrolled for 3 months. Plan of Correction: Provider will obtain physical records.
Standard #: 8VAC20-800-90-A Description: Before a child can attend, the provider shall obtain documentation that the child has been adequately immunized. There was immunization record available for Child 3 who was provided care in July 2024. Child 3 have been enrolled for 3 months. Plan of Correction: Provider will obtain immunization record.
