Technical Assistance:
Technical assistance was provided in the areas of appropriate outdoor materials and notification of menu substitutions.

Effective January 1, 2025, the VDOE will begin determining compliance with ? 22.1-289.057 of the Code of Virginia, which is legislation passed by the General Assembly in 2020. The law requires all licensed child day programs, religious exempt child day centers that serve preschool age children, and certified preschools to test potable drinking water. The law requires that programs submit their plans and test results to the Virginia Department of Health Office of Drinking Water (VDH ODW) and the Superintendent. If the results of the testing indicate elevated lead levels, the program shall remediate, retest, and resubmit results to VDH ODW and the Superintendent. There is an additional alternative bottled water option that comes with additional requirements. The statutory requirement can be found online at https://law.lis.virginia.gov/vacode/title22.1/chapter14.1/section22.1-289.057/.

Comments:
An unannounced mandated monitoring inspection was conducted on 5/23/24 from 9:30 am - 11:15 pm. There were 6 children (16 points) in care with 1 caregiver supervising, within ratios for supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 3 caregiver & household member records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-800-750-B
Description: Based on observation and interview with the provider, topical skin products were not kept inaccessible to children.
Evidence:
1. Two diaper ointments were accessible to children on a low shelf in the playroom.

Plan of Correction: Moved during inspection, we will keep topical creams up high moving forward.

Standard #: 8VAC20-800-870-C
Description: Based on observation and interview with the provider, there is a period of more than three hours between meal times.
Evidence:
1. The posted menu lists morning snack at 8:00 am, and the provider stated lunch was at 12:30 pm. The provider stated they typically have a snack between 10:00 am and 11:00 am, however no snack was served during those hours today.

Plan of Correction: We will ensure that snack is served at the scheduled time, even when the inspector is present.

Description: Based on observation, personally identifying information for the children was not kept privileged and confidential.

Evidence:
1. The Supplemental Information page of the December 7, 2023 inspection was posted. Information found on the Supplemental Information page is considered confidential and not to be posted..

Plan of Correction: We will make sure not to post the supplemental page moving forward.

Description: Based on observation, potentially poisonous substances were not stored in an area inaccessible to children.

Evidence:
1. Hairspray and baby powder (both stating to be kept out of reach of children) were accessible to children on a low shelf in the playroom .

Plan of Correction: Substance was moved during inspection. We will place hazardous substances higher moving forward.

Standard #: 8VAC20-800-290
Description: Based on observation and interview with the provider, sharp objects were accessible to children.

Evidence:
1. A map with push pins was on the wall in the play area, with several push pins on the map within reach of the children.

Plan of Correction: We will raise the map, if the push pins are still within reach we will use stickers moving forward.

Description: Based on observation, children under the age of two years had direct access to stairs.
Evidence:
1. The provider had three children in care under two years of age that had access to stairs in.the playroom. (Ages 12 months, 21 months, and 21 months).

Plan of Correction: We will block the stairs so they are not accessible.

Description: Based on observation, stationary outdoor play equipment was placed over a hard surface.
Evidence:
1. The slide and swing areas of the play structure had hard ground under them. (No grass or other resilient surfacing was present.)

Plan of Correction: We ordered mats to go over under the fall zones.

Standard #: 8VAC20-800-680-A
Description: Based on observation, the provider did not wash their hands with soap and water after diapering a child.
Evidence:
1. The provider finished diapering a child, and wiped her hands with a baby wipe, before diapering another child.

Plan of Correction: We will clean and sanitize the changing pad between diaper changes moving forward.

Standard #: 8VAC20-800-680-B
Description: Based on observation, the provider did not ensure that children's hands were washed with soap and water after diapering.
Evidence:
1. The provider changed one child's diaper and then wiped the child's hands with a baby wipe, and was then released to play.

Plan of Correction: We will make sure each child's hands are washed with soap and water immediately after changing their diaper.

Standard #: 8VAC20-800-690-D
Description: Based on observation, The diapering surface was not cleaned and sanitized after each use.
Evidence:
1. Following a diaper change of child #1, the changing mat was not cleaned with a soap and water solution and then sanitized with a bleach and water solution before another child's diaper was changed.

Plan of Correction: We will ensure that the changing mat is cleaned and sanitized between each use.
