Standard #: 22VAC40-665-540-A Description: Based on review of three staff records, the facility did not ensure that staff were evaluated by a health professional and issued a statement that the individual was determined to be free of communicable tuberculosis (TB) and that documentation of the screening was submitted at the time of employment and prior to coming into contact with children. Evidence: The TB test or screening for Staff A, with a start date of 1/2/20, was dated 1/7/20. Plan of Correction: Moving forward managers will ensure that staff assigned to a subsidy site submit their proof of a tb test or assessment screening prior to start.
