Evidence: When asked for an attendance list of children in care, the provider pulled out a binder with attendance sheets. The most recent date that attendance was documented was 2/3/2023.Plan of Correction: I will start doing it.
Standard #: 8VAC20-800-60-B Description: Based on record review, children's records did not contain all required information.
Evidence: Child #2, Child #5 and Child #6's records did not contain the families medical insurance policy numbers.Plan of Correction: I can ask the parents to give me the numbers.
