Comments:
Licensing Inspector conducted an unannounced Initial Subsidy Inspection on July 13, 2023. There were 17 children ages 2 1/2 to 11 years old present under the direct care of 2 staff. The Licensing Inspector reviewed 4 children?s records and 5 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete.

Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-790-560-A
Description: Based on record review, the Vendor failed to ensure each staff member is evaluated by a health professional and be issued a statement that the individual is determine to be free of communicable tuberculosis.

EVIDENCE:
Licensing Inspector reviewed 5 staff records. Record for Staff #4 did not include documentation of a negative TB skin test/screening. Staff #4 date of hire is 7/05/2023.

Plan of Correction: Staff will complete a TB skin test or a tuberculosis screening from a physician or his designee. Documentation of the results will be maintained in the staff record.

Standard #: 8VAC20-790-790-A
Description: Based on inspection, the Vendor failed to have a written emergency preparedness plan.

EVIDENCE:
Center has not developed a written emergency preparedness plan.

Plan of Correction: Center Director will consult with church administrators to develop an emergency preparedness plan. Upon completion, all staff will receive training and parents of enrolled children will receive a written copy of the plan.
