Technical Assistance:
A model form for an attendance record and emergency drill log will be emailed. This form will be provided in Spanish.

As discussed, the pack and play onsite is only used for play. No children are permitted to sleep in one.

Comments:
An unannounced Subsidy Health and Safety Inspection (SHSI) was conducted today. Observed 2 children ( 6 points) directly supervised by thevendor/provider. Ratio and supervision were in compliance. One child was sleeping and another child was eating afternoon snack. Vendor and inspector reviewed the requirements of emergency drills. Staff and child records were reviewed.
The exit interview was conducted with the vendor/provider. Please provide a corrective actin for each violation and a date to be corrected within 5 working days. Inspection conducted between 1:50pm and 2:45pm. If you have any questions regarding today's inspection, please contact:
