Comments:

An unannounced code compliance inspection was conducted on 04/23/2024 from 10:48 AM to 12:55 PM. There were 72 children present, ranging in ages from infant to five years. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, and procedures. A total of seven children?s records and seven staff?s records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Standard #:	22.1-289.031-B-5
Description:	Based on a review of the staff handbook and interview, the center failed to establish and implement a procedure to ensure that all areas of the premises accessible to children are free of obvious injury hazards, including providing and maintaining cushioning material under playground equipment. Evidence: 1. In the Caterpillar room there was Clorox wipes on top of the low cabinet in the bathroom used by children. 2. In the Kangaroo room there was Lysol wipes on a low shelf. 3. The center's policy is for hazardous substances to be kept out of reach of children. 4. On the school age playground there was exposed ground covering and no mulch under the monkey bars.
Plan of Correction:	The hazardous substances were moved immediately. All staff will be reminded to keep hazardous substances out of reach of children. The ground covering will be removed or covered with rubber mulch. Additional rubber mulch will be added under the bars.
Standard #:	22.1-289.035-A
Description:	Based on record review and interview, the center failed to update background checks every 5 years. Evidence: Staff 2 has a fingerprint background check dated 04/08/2019.
Plan of Correction:	Staff 2 will schedule fingerprints to be completed within 10 days. The date of the appointment will be sent to the licensing inspector. Once the results are received they will be forwarded to the licensing inspector. In the future staff will be required to complete fingerprint background checks before they start work.
Standard #:	8VAC20-770-40-D-2
Description:	Based on record review and interview, the center failed to have staff sign a sworn disclosure statement prior to the first day of employment and have a completed central registry record check by the end of the 30th day of employment. Evidence: Staff 2, first day of employment 2/1/18, and staff 3, first day of employment 2/21/22, do not have a central registry background check in their file. Staff 6, first day of employment 3/27/23, did not have a sworn statement in their file.
Plan of Correction:	Within 10 days the central registry will be completed in the portal and proof will be sent the licensing inspector. When the results are received they will be forwarded to the licensing inspector. In the future staff will complete paperwork for central registry upon hire and the administrator will follow-up to ensure the process is complete. The sworn statement was complete the day of the inspection and the licensing inspector was show the document.
