Comments:

An unannounced, on-site monitoring inspection was initiated on 5/28/24 and the inspection concluded on 5/28/24. The on-site inspection began at 9:15am and ended at 11:40am. The inspector reviewed compliance in the areas listed above. There were 33 children present and 4 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included: ? document review ? tour of the facility ? observations Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations:

Standard #:	22.1-289.035-B-2
Description:	Background fingerprinting results shall be obtained prior to hire for employees. Staff #5 does not have a completed fingerprint letter and was actively working with the children at the time of this inspection.
Plan of Correction:	No staff will be hired without having the fingerprint letter moving forward. Staff #5 will not work with the children again until the fingerprint result letter is obtained. Fingerprints were done on 5/22/24, results are currently pending.
Standard #:	8VAC20-780-160-A
Description:	Staff are to obtain documentation of a negative tuberculosis screening at the time of employment and prior to coming into contact with the children. Staff #2 and staff #3 did not obtain a screening at the time of employment or before coming into contact with the children. The screenings were completed between 1 month and 4 months after hire and working with the children.
Plan of Correction:	Staff will obtain TB screenings prior to hire as required.
Standard #:	8VAC20-780-70
Description:	Documentation of at least 2 references are to be obtained prior to employment. Staff #2, who has been actively working at the center for the past 6 months, did not have any completed reference verifications on file.
Plan of Correction:	References will be obtained for staff prior to hire as required.
Standard #:	8VAC20-780-240-A
Description:	Staff are to complete the VDOE sponsored online orientation course within 90 days of hire. Staff #2, who has been employed and working with the children for 6 months, has not yet completed this course.
Plan of Correction:	Staff will complete the required online training within 90 days of hire as required. Staff #2 will complete this immediately for compliance.
