A self-reported incident inspection was initiated on 6/6/24 and concluded on 6/6/24. The inspector reviewed compliance in the areas listed above.
This inspection included:

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-780-280-H Description: Cosmetics, medications, or other harmful agents shall not be stored in areas, purses or pockets that are accessible to children.
On June 6, 2024 the keys belonging to staff #1 were on the table in the classroom accessible to the children in care. The keys had pepper spray attached. Child #1, 7 years of age, picked up the keys and accidentally sprayed child #2, 8 years of age, in the face.Plan of Correction: Staff items will be in the back office and not on the classroom floor. A verbal training was done that evening to staff. A staff meeting is scheduled for Monday June 17th as well.
Standard #: 8VAC20-780-340-A Description: When staff are supervising children, they shall always ensure their care, protection, and guidance.
On June 6, 2024 the keys belonging to staff #1 were on the table in the classroom accessible to the children in care. The keys had pepper spray attached. Child #1, 7 years of age, picked up the keys and accidentally sprayed child #2, 8 years of age, in the face. Child #2 required first aid intervention to relieve the affects of being pepper sprayed in the face. Staff #1 did not ensure the care, protection, and guidance of the children by leaving the keys, with pepper spray attached, accessible to the children within the classroom.Plan of Correction: Director and staff will ensure that children in their care will be cared and protected from harm. A staff meeting is scheduled for Monday June 17th as well.
