Technical Assistance:
Discussed the importance of keeping attendance. Discussed background checks for future household members.

Comments:
An unannounced focused inspection was conducted today from 9:35am-10:15am with the provider on site. The caregiver was over points from approximately 9:35am to 9:51am with 6 children (17 points) until the second caregiver arrived.. One child's file was reviewed. The children were engaged in the following: participating in circle time, playing with toys and puzzles. The physical space, attendance records and the Emergency Preparedness Plan were reviewed. There
were no medications on site. Areas of non-compliance are identified in the violation notice. Repeat violations were found and are identified within this report.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to the department within 5 business days from receipt. Please specify how the deficient practice will be or has been corrected. Please do not use caregiver
names; list caregivers by positions only.

Any plans of correction not received within 5 business days will result in the plans not appearing on the Violation Notice and public website.

Standard #: 8VAC20-800-50-A
Description: Repeat Violation
Based on review of the attendance record and interview, the family day home did not keep a
written record of children in attendance each day.
Evidence:
1. At approximately 9:35am, there were 6 children in care but only four children were
listed on the attendance sheet. The provider confirmed the two children not on the
attendance sheet arrived at approximately the following times:
Child #5-8:30am
Child #6-8:30am

Plan of Correction: From now on I'll make sure the attendance record has all children's name.

Standard #: 8VAC20-800-540-A
Description: Repeat Violation
Based on observation, high chairs were not being used to the manufacturer's instructions and when occupied by a child, a safety strap was not being used and securely fastened.
Evidence:
1. At approximately 9:35am, Child #4 (3 year old) was observed in a high chair without a safety strap on.

Plan of Correction: I'll make sure to have safety strap secure when children are using the high chairs.

Standard #: 8VAC20-800-570-A
Description: Based on observation, the provider did not ensure that a caregiver does not exceed 16 points by using the point system to determine if an additional caregiver is needed.
Evidence:
1. One caregiver was caring for 6 children (17 points) from approximately 9:35am to 9:51am and was exceeding 16 points. The following children were in care and their ages/points assigned to the age :
Child 1 *3 years old (2 points)
Child 2 *20 months old (3 points)
Child 3 *9 months (4 points)
Child 4 *3 years old (2 points)

Plan of Correction: I'll make sure to have another assistant if I'll be absent from daycare. I'll also make sure to have a complete folder for the extra assistant.
