Comments:
An unannounced complaint investigation was conducted on 6/22/22 in response to a complaint received in the licensing office regarding allegations of the center not reporting a serious injury immediately; unsafe physical plant; care, protection, and guidance of children; a child not being changed when their clothes became soiled; improper behavioral guidance; and staff: child ratios. Based on a review of documentation and interviews conducted, the allegations regarding immediately reporting of a serious injury; unsafe physical plant; and care, protection, and guidance of children were determined to be valid. The allegations regarding a child not being changed when their clothes became soiled; improper behavioral guidance; and staff: child ratios were unable to be substantiated due to insufficient corroborating evidence. Description: Based on observation, the center did not ensure that areas and equipment of the center, inside and outside, were maintained in a safe and operable condition. Evidence: 1. A metal post on the side of the gate that goes around the playground had become warped to the point of bursting open, leaving a sharp opening in the post that was several inches long and within reach of children. 2. Around the top of the fence in multiple areas, the metal fencing was pulling away from the metal bar that was meant to hold the fencing upright. The fencing was able to be pulled away from the bar and was sharp in some places.

Plan of Correction: A call was made to maintenance on the day of this visit and all work
was completed prior to receiving the notice on 7/20/2022.

Complaint related: Yes
Description: Based on interview, staff who were supervising children did not always ensure their care, protection, and guidance. Evidence: On 6/3/22 at approximately 11:00am a three-year-old child vomited one time and the center called the child?s guardian to report the illness. When the guardian arrived, staff reported that the child had stated that they had ?eaten bird poop off of a stick? while on the playground prior to vomiting.

Plan of Correction: Prior to children going to the playground, a staff member will walk the
playground to pick up any sticks from the grassy areas, and children will be reminded to not touch anything with bird feces. Notification to a parent/guardian will happen if a child comes into contact with any bird feces. Playground Safety and Supervision training will be retaken.

Description: Based on review of documentation and interview, the center did not ensure that they notified a parent immediately when a child sustained a serious injury. Evidence: On 6/1/22 at approximately 8:50am a child bumped their forehead on playground equipment, resulting in a large lump/bruise with a cut in the middle. The center sent a picture of the child to the guardian through the online portal doing an activity at approximately 10:00am. The guardian viewed the picture at approximately 1:05pm and called the center to ask about the injury. Center staff reported that they had been very busy and had not been able to contact the guardian yet.

Plan of Correction: Staff will be trained on sending the parent/guardian a message through the
parent engagement app on classroom tablets to notify the parent/guardian of an accident when access to the center phone is unavailable due to maintaining ratio. Staff will continue to document all injuries on paper at the time they occur.
