An unannounced subsidy health and safety inspection was conducted on 5/9/2024 beginning at 9:04am and ending at 10:25am. There was 1 child in attendance at the time of the inspection. The child was observed participating in reading time, music time, and toy play with the vendor. Required background searches, training certificates and household member information was reviewed. A sample of requirements for daily attendance, child information/records, emergency preparedness planning and supplies were also reviewed. The information gathered during the inspection determined non-compliance with applicable standards or laws and violations were documented on the violation notice issued to the vendor. Standard #: 8VAC20-790-190-B Description: Based on review of children's information, name, address, and telephone number of at least one person designated by the parent to contact in case of an emergency if the parent cannot be reached was not collected by the vendor.
Evidence: There were no emergency contacts for child #4.Plan of Correction: Vendor will obtain emergency contact information for child #4.
Standard #: 8VAC20-790-190-B Description: Based on review of children's information, telephone number for each custodial parent's place of employment was not found.
Evidence: Telephone numbers for each custodial parent's place of employment was not found for child #1 and child #4.Plan of Correction: Vendor will obtain work phone numbers for the parents of the children.
