Evidence -
1. On the date of inspection (06/18/2020), the information was not complete for a total of two emergency contacts listed for each child: Child #1 had no contacts listed; and Child #2 had 1 of 2 contacts listed.
2. The written authorization for emergency medical care was not signed by the parent of Child #2. This area was left blank.Plan of Correction: All missing information shall be completed. Authorization shall be signed.
