Technical Assistance:
Today we discussed Fall Zones to include the types of equipment which require them and the depth of resilient surfacing to include raking and fluffing to keep it from compacting.

Also discussed was alternatives to initially giving every child more than one measured serving for each food component at meal time. Since some of the children do not eat "any" of some food components, it may be more efficient (and allow children who are more hungry to have more additional servings) if you provide one serving of each food to each child and put extras in bowls for providing additional servings to children who want them.

Comments:
A monitoring inspection was initiated and concluded on 5/8/2024. There were 20 children, ages 10 months to 4 years of age present with 4 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. Children were observed washing hands and eating lunch which consisted of macaroni and cheese, green beans, mandarin oranges, and milk. Other activities observed were story time, planting seeds in cups of soil, music and movement, and rest time. Infants were observed being held, rocked and bottle fed, having tummy time, crawling on the carpet and resting in cribs. A total of 5 child records and five staff records were reviewed. The inspection started at 11:10 am and concluded at 1:55 pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-150-B
Description: Based on record review, the licensee did not ensure that enrolled children's required physicals and immunization reports are always signed or stamped by a physician, his designee, or an official of a local health department.
Evidence:
Five children's records were reviewed.
The record for child #1 contained an immunization record which was not signed (or stamped) by a physician, his designee, or an official of a local health department.
The record for child #2 contained the results of a physical examination which were not signed (or stamped) by a physician, his designee, or an official of a local health department.

Plan of Correction: Parents will be reminded this week that physicals and immunizations must be signed or stamped by the child's a physician, his designee, or an official of a local health department.
The parent s of child #1 and child #2 will be asked to provide a signed copy of their immunizations and physical results for their child's file within 2 weeks.

Standard #: 8VAC20-780-330-B
Description: Based on observation and interview, the licensee did not ensure that where playground equipment is provided, resilient surfacing shall comply with minimum safety standards when tested in accordance with the procedures described in the American Society for Testing and Materials standard.
Evidence:
A blue two-seat plastic rocker was located on the playground without resilient surfacing in the fall zone / under and around it.
The rocker seats were approximately 12 inches from ground level.

Plan of Correction: The blue rocker was removed from the outdoor play area during the inspection and will not be used by children unless or until the required resilient surfacing is installed in the fall zone for the equipment.
