Comments:
Thank you for your time and assistance with the unannounced monitoring inspection conducted by the inspector and Licensing Administrator on January 14, 2020. The on-site inspection was from 10:05 a.m. to 1:35 p.m. On this date, there were 22 children present in three classrooms and in the care of three staff members. The program administrator and one volunteer were also available to assist, if needed. The total enrollment for the program was 62 children. The exceptions to compliance were noted on the violation notice and the risks ratings were noted on the supplemental page. Four staff members' records and five children's records were reviewed. Three classrooms were toured and observations were made in two of those rooms. Four staff members and six children were interviewed. No medication is being administered at this time. All interactions between children and staff were age appropriate and positive. Some items that were reviewed and observed included: required posted information, injury reports, documentation of evacuation, shelter-in-place, and lockdown drills, emergency supplies, first aid kit, warm water temperature, updated emergency preparedness plan, art activities, floor puzzles, water play, reading station, kite flying, hand washing and bathroom procedures, transitions, lunch, and nap time.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 10 calendar days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word ?corrected? is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on review of 5 children's records, program failed to include required information in children's records.

Evidence: 1. The records of Child #1 and Child #4 did not include documentation of immunizations. 2. The records of Child #1 and Child #2 did not include documentation of physicals within 30 days of attendance. 3. Record of Child #5 contained parental agreement that was unsigned. 4. Program administrator confirmed information was missing and that it was not filed elsewhere.

Plan of Correction: Parents notified and forms provided.

Standard #: 22VAC40-185-340-F
Description: Based on observation, the program failed to provide intermittent sight supervision of the children in the restroom area during the five-minute period to assure the safety of children and to provide assistance to children as needed.

Evidence: 1. Licensing Administrator observed two children that were using the restroom. One child had exited a stall to wash hands, when another child exited to play with other child, prior to pulling pants up. 2. Licensing Administrator alerted the program administrator, and she intervened.

Plan of Correction: Retrained staff on sight and sound supervision. Take the entire group to wash hands instead of individually.
