Comments:
An unannounced inspection was conducted on May 22, 2024 from 9:35 a.m. to 12:00 p.m. to investigate a self-reported incident regarding sight and sound supervision. Staff interviews were conducted. Documentation was reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-340-F
Description: Based on staff interviews and review of video footage, the center failed to ensure that children under the age of 10 years were in sight and sound supervision at all times.
Evidence: On Wednesday, May 15, 2024, Child #1, two years of age, was left alone on the playground for approximately one minute.

Plan of Correction: On May 16, 2024 staff were provided with professional development regarding active supervision including zoning, scan and count, and name to face.
Use of the clipboard with child name and time signed in was reviewed along with the expectation that the clipboard would be taken any time children left the classroom.
Staff were trained on adult placement when transitioning in and out of the room, one in front of line, one at end of line and head count needed to be stated out loud and seconded.
A laminated board was placed at both exits of the classroom for count to be written on as a reminder of how many students are present with the expectation that this be reviewed and updated every hour for accuracy.
