Comments:
An unannounced complaint inspection was initiated by the inspector in the center from 11:50AM until 1:00PM on May 30, 2024 and completed on June 27, 2024, regarding concerns of Staffing/ Supervision, Special Care Provisions/Emergencies and Programs. There were 21 children present with 2 staff supervising. During the inspection the following were completed: interviews with administrative staff, interviews with center staff, a review of center records/documentation and observation of children their program space.

The preponderance of the evidence gathered during the investigation does support the allegation related to staffing and supervision; therefore, the complaint is determined to be valid. The inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Description: Based on staff interview and observation, the center did not maintain required staff to child ratios, 1 staff for every 5 children, as an extension of a temporary staff-to-child ratio variance granted by the Office of Child Care Health and Safety on September 21, 2023.

Evidence: During the inspection conducted on May 30, 2023 from 11:50AM to 1:00 PM, in the infant room, Staff #1 was observed alone with 7 infants (youngest, Child #1, age 7 months). The staff-to-child ratio for children birth to 16 months is 1:4 but the center had an approved variance for 1:5. Center owner confirmed Staff #1 was the only staff working with the infant children on May 30, 2024.

The staff-to-child ratio for children two year old is 1:8 but the center had an approved variance for 1:9. Center owner confirmed Staff #2 was the only staff working with the children ages 2-4 on May 30, 2024.

Plan of Correction: We will refuse any excess children if adequate staff calls and quits or is not available
