Comments:

An unannounced monitoring inspection was conducted on 7/10/2024. There were two licensing staff on site from approximately 9:57am until 11:05am. There were four children present with one caregiver supervising, totaling 10 points. Children were observed in variety of activities including reading, playing with play food, playing with building blocks and playing board games. Caregiver interacted with children in a positive manner. Licensing staff interacted with caregiver and children in care. The inspectors reviewed compliance in the areas of administration, personnel, household members, caregiver training, physical equipment and environment, emergencies, nutrition, medication administration, and care of children. A total of five child records, one caregiver record and one household member record were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Description:	Based on a review of children?s records, the provider did not obtain documentation that the child has been adequately immunized according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations before the child attended the family day home. Evidence: 1. The record for child #3 (enrolled 2/12/2024) contained immunizations that were received and signed by a physician on 2/16/2024. 2. The provider acknowledged that the immunizations were received late.
