22.1 EARLY CHILDHOOD CARE AND EDUCATION

A monitoring inspection was conducted on May 23, 2023. The inspectors were on site from approximately 2:00 pm until 4:45 pm. The director was present during the inspection. There were 39 children present, ranging in ages from 11 months to eleven years, with seven staff supervising. Upon the inspector?s arrival, children were napping. After nap, children were observed having snack, finger painting, and playing a game in circle time. Later school-age children arrived and washed their hands before sitting done to snack. School-age children also worked on homework, word puzzles and coloring. Interactions between staff and children were positive.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and six staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law. Violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-780-60-A Description: Based on review of five children?s records and interview, the center did not obtain all required information for three children?s records.
Evidence:
1. The record of child # 2, enrolled 04/30/2024, did not contain the work phone number and place of employment of each parent who has custody.
2. The record of child # 3, enrolled 08/03/2021, did not contain the work phone number and place of employment of each parent who has custody.
3. The record of child #5, enrolled 06/01/2021, did not contain the work phone number and place of employment of each parent who has custody.
4. The child record form for child #2, child #3, and child #5 did not request the work phone number and place of employment from each parent with custody. The director acknowledged the child record forms did not ask for the employment information.Plan of Correction: Per the director: I believe i accidently used the old child's record forms. I will have parents update the missing information and ensure I am only using the current child record form that includes all the required information.
