Comments:

An unannounced monitoring inspection was conducted on September 4, 2019 from approximately 9:15am-12:40pm. There were no children present but the center has a total enrollment of 58 children at this time. Three classrooms, a gross motor room and the outdoor playground area were inspected. Six children's records, six staff records and required written information, policies and procedures were reviewed. The center does not offer transportation.

Violations:

Standard #:	22VAC40-191-40-D-2
Description:	Based on record review, in six of six staff records reviewed, the center did not ensure that employees have a Sworn statement or affirmation before employment or commencement of service at the facility and/or a Search of the central registry within 30 days of employment or commencement of service. Evidence: The Director confirmed that all six staff records reviewed did not have results of a Search of the Central Registry and that the five following staff members did not have a sworn statement completed prior to employment: Staff #2 (date of hire 8/20/18) Staff #3 (date of hire 6/9/19) Staff #4 (date of hire 8/20/18)-Sworn Statement dated 8/19/19 Staff #5 (date of hire 6/5/19) Staff #6 (date of hire 6/9/19)
Plan of Correction:	The necessary documentation will be obtained for all necessary staff members. The intake procedures for new hires will be immediately modified to ensure the timeliness of the required documentation.
Standard #:	63.2(17)-1716-B-7
Description:	Based on inspection and staff interview, the center had not established and implemented all required procedures regarding reports of serious injury or death to children attending the child day center. Evidence: The Director confirmed that the following procedures had not been established and implemented: 1. Ensuring that all incidents involving serious physical injury to or death of children attending the child day center are reported to the Commissioner. 2. Reports of serious physical injuries, which shall include any physical injuries that require an emergency referral to an offsite health care professional or treatment in a hospital, shall be submitted annually. 3. Reports of deaths shall be submitted no later than one business day after the death occurred.
Plan of Correction:	The aformentioned policies and procedures will be reviewed with staff and posted in the staff closet.

Description:	Based on record review, in four of six staff records reviewed, the center did not ensure that staff submit to fingerprinting and obtain results prior to employment. Evidence: The Director confirmed that the fingerprint result letters for the following staff members were obtained after the hire date: Staff #1 (hire date 5/1/18)-fingerprint results letter dated 8/16/18 Staff #3 & Staff #6 (hire date 6/9/19)-fingerprint results letter dated 6/11/19 Staff #5 (hire date 6/5/19)-fingerprint results letter dated 6/11/19
Plan of Correction:	The intake procedures for new hires will be immediately modified to ensure the timeliness of the required documentation.
