Comments:

An unannounced monitoring inspection was conducted on 02/18/2020. There were 4 children, ages 2 years-4 years, totaling 8 points and one provider present during the inspection. The inspector reviewed 5 children's records, one provider record, one household member record and 3 pet records during the inspection. The provider stated that medication is not administered. The children were observed in the following activities: free choice activities, watching an educational show and during lunch. The inspector reviewed the emergency supplies during the inspection. The inspector discussed nap time with the provider and reminded the provider of upcoming expirations and necessary updates. Description:	Based on record review and discussion, the provider failed to obtain documentation of additional immunization for children once every six months for children under the age of 2 years. Evidence: The record for Child #5 did not contain documentation of additional immunizations once every six months. Child #5 was under the age of 2 years until 12/2020; the most recent additional immunization was dated 06/17/2019,
Plan of Correction:	The provider call Child #5's parent during the inspection and requested a copy of the most recent immunizations.

Description:	Based on observations and discussion, the provider failed to ensure that child-resistant protective covers shall be installed on all unused electrical outlets and surge protectors accessible to children under five years of age. Evidence: There was an uncovered electrical outlet in the restroom that is used by children. The uncovered electrical was accessible to the children.
Plan of Correction:	The outlet was covered during the inspection.
