Evidence: Staff #1 (Date of Hire: 8/31/2020) has a signed Sworn Disclosure Statement on file dated 9/30/2020.Plan of Correction: Will make sure our new hire staff packets have all needed documents upon hiring process.
Standard #: 22VAC40-191-60-C-2 Description: Based on record review, the center did not ensure that all staff have the results of a Virginia central registry search on file within 30 days of employment.
Evidence: Staff #1 (Date of Hire: 8/31/2020) does not have central registry results on file. Documentation in the staff's file states the central registry search request was sent on 10/1/2020.Plan of Correction: Will make sure CPS is mailed off 1st 7 days of employment.
