63.2(17) License & Registration Procedures

Comments:
An unannounced Code compliance inspection was conducted today, 9/25/19. Upon arrival to the center the children were in the upstairs classroom in center play. Snack and hand washing was also observed in this area. The children are learning to write. Outside play, lunch and nap time was also observed. A review of staff and children's records was also conducted.
We discussed the divisions web site and how to access the information regarding background checks and memos that have been sent out by email.
There are 11 children in care ranging in age from 2 1/2-5 years with two staff present.

Violations:
Standard #: 22VAC40-191-40-D-2
Description: Based on review of records and interviews the center failed to have all the required background checks on the staff.
Evidence:
1. Staff # 1-3 do not have a Central Registry (CPS) record check on file. All staff have been employed longer than 30 days.
2. Staff # 1-3 do not have a Sworn Disclosure or affirmation statement on file.

Plan of Correction: The director will ensure that all staff and substitutes will have all the required background checks on file.

Standard #: 63.2(17)-1716-A
Description: Based on review of written information provided to the parents the center failed to provide all the required information.
Evidence:
1. The written information provided to parents/guardians did not include qualifications of personnel.

Plan of Correction: The director will include this information in the parent handbook.

Standard #: 63.2(17)-1716-A-6
Description: Based on review of written information provided to the parents the cenrer failed to provide all the required information.
Evidence:
1. The parent handbook did not include information about staff health requirements or public liability insurance.

Plan of Correction: The director will ensure that this information is provided to the parents/guardians in writing.

Standard #: 63.2(17)-1720.1-B-2
Description: Based on review of staff records and interviews the center failed to have all the required background checks on file.
Evidence:
1. Staff # 1-3 do not have a fingerprint-based criminal history check determination letter prior to he first day of employment. All staff have been employed more than 30 days.

Plan of Correction: The director will ensure that all staff have a finger-print background check through Fieldprint.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on interviews the center failed to obtain an out-of-state child abuse and neglect search on staff.
Evidence:
1. Staff #2 reported living outside of the state of Virginia in the past five years and has been employed more than 30 days and there are no child abuse and neglect checks from that state.

Plan of Correction: The director will ensure that all staff have all the required background checks within the set time frames.
