Comments:
An unannounced monitoring inspection was conducted May 14 2024 between 9:15am and 10;15am. There were no children in care at the time of the inspection. Child records, household records, the provider records, evacuation drills, and medication training was reviewed and inspected. A citation was issued and is listed on the violation notice. Description: Based on record review, the provider did not ensure that each child record contains the required documentation.

Evidence: Child #2 record does not contain the addresses of the emergency contacts listed on the Child Record form.
