Comments:
An unannounced monitoring inspection, required by the Provisional License, was conducted on 06/25/2024. There were 69 children, ages 14 months- 13 years, and 9 staff members present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, staffing and supervision, physical plant, programs, emergencies and nutrition. The children were observed during free choice activities, in learning activities, and in outside large motor play. A total of 6 children?s records and 6 staff records were reviewed. The inspector arrived for the inspection at 8:55 am and departed at 10:55 am.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Violations:
Standard #: 8VAC20-780-330-B
Description: Based on observation, the center failed to ensure that where playground equipment is provided, resilient surfacing shall comply with minimum safety standards and shall be under equipment with moving parts or climbing apparatus to create a fall zone free of hazardous obstacles.

Evidence: The moveable small red slide on the playground was located over dirt and grass and not over an acceptable form of resilient surfacing; children were observed using the slide during the inspection.

Plan of Correction: The slide was removed from the playground area.

Standard #: 8VAC20-780-350-B-4
Description: Based on observation and record review, the center failed to maintain the required staff-to-child ratio whenever children are in care.

Evidence: There were 35 children and 2 staff members observed with the school-age group of children at approximately 9:00 am during the 06/25/2024 inspection. Child 7 was determined to be 4 years old and will not be school-age eligible by September 30. The required staff-to-child ratio for children 3 years old up to school age eligible is 1:10. Four staff would be required for this group of children.

Plan of Correction: The child will be moved to the pre-k class.

Standard #: 8VAC20-780-420-E-1
Description: Based on staff interviews, the center failed to ensure that for each infant, the center shall post a daily record which can be easily accessed by both parent and the staff working with the child that contains all of the elements required by the standard.

Evidence: Staff 7 confirmed that there was not a daily record for the 2 infants in care on 06/25/2024.

Plan of Correction: Will start using the forms again.

Standard #: 8VAC20-780-500-C
Description: Based on observations, the center failed to ensure that toilet chairs shall be emptied promptly and cleaned and sanitized after each use.

Evidence: There was a urinal style toilet chair in the restroom that contained urine. The toilet chair was not promptly cleaned and sanitized after each use.

Plan of Correction: Will retrain the staff and will post a sign to remind them to empty, clean and sanitize the potty chairs.
