Comments:
A complaint inspection was conducted with an outside agency beginning on February 13, 2024, February 28, 2024, March 21, 2024, April 4, 2024, May 1, 2024 and concluded on May 23, 2024. A complaint was received at the Office of Child Care Health and Safety (Valley) regarding child abuse and neglect. The inspector reviewed compliance in the areas of administration, staff and supervision, programs, and background checks. Six interviews were conducted, attendance records, staff schedule and one child record was reviewed.

The preponderance of the evidence gathered during the investigation DOES NOT support the allegation; therefore, the complaint is determined to be not valid. In addition to the original complaint, there were also violations found during the inspection regarding mandated reporting abuse and neglect. See the violation notice for more details.

Please complete the plan of correction and date to be corrected sections for each violation cited on the violation notice and return it to me within 5 business days from today. Which will be close of business on Thursday, June 13, 2024. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s): 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Description: REPEAT VIOLATION

Based on document review and interviews, staff who had reason to suspect that a child is abused or neglected failed to report the matter immediately to the local department of the county or city wherein the child resides or wherein the abuse or neglect is believed to have occurred or to the Departments toll-free child abuse and neglect hotline.

Evidence:
1. The Program Director received two photographs of bruises on both arms of Child 1 (five years of age) on June 30, 2023 that were reported as a possible result of Child 1 being grabbed while in care of the child day center. The Program Director and at least two other staff were aware of these bruises and failed to report the incident to the local department.

Plan of Correction: The local department of Social services will come and train all our staff on abuse and neglect reporting. We will use this guidance to change our policies and practice.
