Comments:
An unannounced code compliance inspection was conducted on 06/26/2024 from 11:50 a.m. to 1:10 p.m. There were 40 children present, ranging in ages from 11 months to 12 years old, with 10 staff. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, procedures, and medication. Five children?s records and five staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 22.1-289.031-B-4
Description: Based on review of records and interview, the center failed to obtain immunization records before the first day of attendance that were signed and dated by a physician.
Evidence:
1. Child #1?s first day of attendance was 02/20/2024. The immunization record on file dated 02/20/2024 was not signed by a physician.
2. Child #3?s first day of attendance was 03/18/2024. The immunization record on file dated 03/17/2024 was not signed by a physician.
3. Child #4?s first day of attendance was 04/17/2024. The immunization record on file dated 01/26/2024 was not signed by a physician.

Plan of Correction: attached are the updates

Standard #: 22.1-289.035-B-4
Description: Based on review of records and interview, the center failed to complete an out of state sex offender search for each state a staff has resided in the past five years prior to the first day of employment and the center failed to request an out of state child abuse and neglect search for each state a staff has resided in the past five years by the end of the 30th day of employment.
Evidence:
1. Staff #1?s first day of employment was 10/03/2022. There was not an out of state sex offender search on file for Staff #1. Staff #6 confirmed that the out of state sex offender search was not on file for Staff #1.
2. Staff #5?s first day of employment was 06/05/2023. There was not an out of state sex offender search or an out of state child abuse and neglect search on file for Staff #5. Staff #6 confirmed that the out of state sex offender search and the out of state child abuse and neglect search was not on file for Staff #5.

Plan of Correction: I ran the sex offender. Its attached on Staff #1.
Staff #5 only works in the summer. I did run sex offender but didn't print. It is now printed and it is now printed and I mailed out of state abuse.

Standard #: 8VAC20-770-40-D-2
Description: Based on review of records and interview, the center failed to obtain central registry results by the end of the 30th day of employment.
Evidence:
1. Staff 5?s first day of employment was 06/05/2024. There were no central registry results on file for Staff #5. Staff #6 confirmed that central registry results had not been obtain by the 30th day of employment for Staff #5.

Plan of Correction: I did send off for central reg as I showed the inspector but have not received results. Also she had me send anther email and I still have not received anything. Attached what I showed the inspector.
