Comments:

An unannounced monitoring inspection was conducted on Friday, October 25, 2019 to determine the center's compliance with licensing standards that could not be determined during the initial inspection. The inspector arrived at the center at 3:45pm and concluded the inspection at 5:40pm. The center's director assisted the inspector throughout the inspection. The census for today's inspection consisted of 33 children in the direct care of two staff members. The children were observed during a variety of activities to include playing with toys in their designated area in the cafeteria, playing in the playground, completing art activities, reading books, and taking bathroom breaks. The staff were observed having positive interactions with the children. Per the director, there are no medications on site at this time. Five children's records and information required to be kept at the center for staff were reviewed. Description:	Based on a review of five children's records and interview on 10/25/2019, the center did not ensure that five children's records contained the required information. Evidence: 1) The record for Child #3, enrolled on 09/16/2019, did not contain an address the for the two emergency contacts listed. 2) The record for Child #5, enrolled on 09/09/2019, did not contain an address for the two emergency contacts listed. 3) The records for Child #1, Child #2, Child #3, Child #4, and Child #5, did not contain each child's first date of attendance. The record for each child enrolled shall contain the name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached. 4) During interview, a member of management reviewed attendance records and confirmed the first dates of attendance for Child #1, Child #2, Child #3, Child #4, and Child #5. The record for each child enrolled shall contain the first and last dates of attendance.
Plan of Correction:	Per the Center: "Children's files will be updated to include first dates of attendance. The director will obtain the missing emergency contact information for each child."
