Comments:
An unannounced monitoring inspection was conducted today, March 20, 2024, to determine the centers compliance with licensing standards. The inspector was on site from approximately 3:30 pm until 5:00 pm. The site director and regional director were present during the inspection. Upon the inspector?s arrival there were 21 children in care with three staff supervising. Children ranged in age from six years to eleven years. Children were outside playing basketball and on the equipment. Later children had time in the gym where that had relay races and played with hula hoops. Children ranged in age from five years to eleven years.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. Staff interacted with children in a positive manner. A total of five child records were reviewed. Four staff records were reviewed remotely on March 22, 2024. The inspector interacted with staff and children in care.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-60-A
Description: Based on a review of five children?s records and interviews, the center did not ensure three children?s records contained the required information.

Evidence:
1. The record of Child #3 did not contain the work phone number and place of employment of each parent who has custody and the name, address, and phone number of a second emergency contact to call in an emergency if a parent cannot be reached.
2. The records of Child #4 and Child #5 did not contain the work phone number and place of employment of each parent who has custody.

Plan of Correction: Per the administrator: they will alert BOT team and follow up with the parents.

Standard #: 8VAC20-780-60-A-8
Description: Repeat
Based on a review of documentation and interviews, the center did not obtain a written care plan for one child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.

Evidence:

The record of Child #6 indicated the child had a diagnosed food allergy, but the child?s record did not contain a written allergy care plan. During interview, a member of management confirmed the center does not have an allergy care plan for Child #6.

Plan of Correction: Per the administrator: they will alert BOT team and follow up with the parents.

Standard #: 8VAC20-780-240-A
Description: Based on review of four staff records, the center did not ensure one staff completed the Virginia Department of Education- sponsored orientation course within 90 calendar days of employment.

Evidence:

The record of staff #2, employed 02/05/2023, contained documentation of the Virginia Department of Education-sponsored orientation course dated 09/26/2023, exceeding 90 calendar days from employment.

Plan of Correction: Per the administrator: staff will complete the training within 90 days going forward.
